Requirements
- Be proficient with a personal computer.
- Have technical and analytical skills.
- Have strong communication, organizational, and interpersonal skills.
- Microsoft Excel experience is preferred.
- A bachelor’s degree is preferred; equivalent professional experience will be considered.
- Be able to work remotely from home in a private HIPAA-compliant workspace.
- Be able to house the company equipment needed to perform the job.
- Have broadband internet with download speed of at least 24 Mbps and upload speed of at least 4 Mbps.
- Maintain regular, reliable attendance during scheduled hours.
Responsibilities
- Develop relationships with patients through telephonic outreach after a clinical event or diagnosis.
- Collaborate with the clinical team and use health-system resources to support the patient experience.
- Research and identify community resources that can assist patients.
- Help patients communicate their preferences and priorities to their healthcare team.
- Support patients in taking an active part in healthcare decision-making.
- Contribute to the development, implementation, and evaluation of the Care Guide navigation program.
- Help address non-clinical factors affecting patient health.
- Meet productivity expectations for timely, high-quality delivery.
