Requirements
- Hold a Bachelor of Science Degree in Nursing.
- Have an active, unencumbered multistate RN license through the Nurse Licensure Compact.
- Hold additional active RN licenses in non-compact states, such as California, New York, and Illinois.
- Have at least 3 years of nursing experience, including 2 years in oncology nursing.
- Be comfortable using Slack, Zoom, Google Suite, and EHR systems.
- Have Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) certification, or obtain a relevant certification within 2 years of hire.
- Hold Basic Life Support (BLS) certification by the start date.
Responsibilities
- Provide consultative clinical support to patients by phone, email, and video, and respond to urgent clinical escalations.
- Triage Symptom Survey responses and other reported symptoms, supporting patients directly or escalating to a Nurse Practitioner.
- Identify patient needs and coordinate care, nutritional support, discharge planning, and provider referrals.
- Educate patients about their diagnosis, Daymark Health's care offerings, and appropriate next steps.
- Build trusting relationships with patients and their care networks through empathetic, active listening.
- Coordinate with Nurse Practitioners, Medical Directors, Social Workers, and non-clinical Health Partners.
- Follow clinical protocols and company policies, procedures, and documentation standards.
- Work with Clinical Operations to use systems, tools, and technology effectively.
