Requirements
- High school diploma or equivalent required.
- Associate or bachelor's degree in healthcare, social services, business, or a related field preferred.
- 1–2 years of experience in care coordination, patient support, customer support, or case management.
- Strong written and verbal communication skills for interacting with patients, providers, and facility stakeholders.
- Strong organizational skills and ability to manage multiple patients, tasks, and competing priorities.
- Ability to work independently, follow established processes, and escalate issues appropriately.
- Strong attention to detail and commitment to accurate documentation and follow-through.
- Problem-solving and critical-thinking skills to identify barriers and determine appropriate next steps.
- Ability to handle sensitive patient interactions with professionalism, empathy, and discretion.
- Comfort navigating multiple systems and platforms.
- Experience working directly with patients or members in a healthcare or service-oriented environment preferred.
- Experience with healthcare, behavioral health, telehealth, or provider networks preferred.
- Familiarity with patient engagement, care transitions, or coordination between levels of care preferred.
Responsibilities
- Monitor patient requirements and paperwork, follow up on outstanding items, and help ensure accounts are activated and ready for care.
- Provide patients with information about next steps, appointments, and care coordination processes, and respond to their questions and concerns.
- Monitor patients during their initial care journey and conduct outreach during the first 30 days to support continued engagement.
- Identify patients at risk of disengagement, follow up on missed appointments, and coordinate with internal teams to address barriers to care.
- Follow established processes to help prevent incorrect billing for missed first appointments and escalate issues as needed.
- Coordinate with Talkspace providers and patients to facilitate transitions to an appropriate UHS facility when higher-level care is needed.
- Contact UHS facilities to coordinate scheduling, communicate next steps, and track transition activities through completion.
- Document outreach, engagement, and care transitions, collaborate with internal teams, and escalate complex issues to the Care Coordinator Manager or clinical team.
