Requirements
- Have prior experience coordinating or navigating care for patients, caregivers, and their loved ones.
- Provide a personal computer or laptop that meets security and telehealth standards (SOC2).
- Be able to spend more than 70% of the day on the phone connecting with and coordinating care for patients and caregivers.
- Demonstrate empathy, patience, time management, and communication skills.
Responsibilities
- Answer inbound calls, emails, and text messages from patients, caregivers, families, external providers, and other facilities.
- Schedule appointments and maintain contact with patients by phone or email.
- Explain health insurance benefits and eligibility, and process copays and coinsurance as needed.
- Follow up after appointments to provide next steps and coordinate care.
- Ask screening questions and document patient safety, behavior, function, medications, care needs, and advanced care plans under clinical supervision.
- Ask screening questions and document caregiver issues, including burden, depression, and poor coping, under clinical supervision.
- Report screening results and escalate concerns to the clinical team.
- Send educational materials and information about Isaac Health care and local community-based services indicated in the patient’s care plan.
- Provide emotional and psychological support to patients, caregivers, and families, and answer questions within the scope of practice.
