Requirements
- Hold an active Community Health Worker (CHW) certification in the state of Texas.
- Be bilingual in English and Spanish, spoken and written.
- Have 3+ years of relevant community outreach, facilitation, or healthcare experience.
- Have strong computer skills and be able to navigate web-based and app-based systems.
- Understand socio-economic and public health challenges facing disenfranchised populations in Texas.
- Have a high school diploma or GED; a college degree is preferred.
- Have a valid driver’s license and reliable transportation.
- Have a flexible schedule for occasional work outside regular business hours.
- Be willing to complete and maintain CMS-mandated training and state-applicable requirements.
- Be able to drive to public places and member homes within a 50-mile radius of the listed Texas locations.
- Be able to work remotely with high independence and minimal oversight.
- Be able to use a computer, tablet, and smartphone to update data accurately in multiple secure systems.
- Frequently carry up to 30 pounds of supplies and stand or walk for the duration of home visits or member interactions.
Responsibilities
- Execute person-centered care plans for patients with serious, high-risk illnesses and assist with monitoring and revising disease-specific plans.
- Coordinate care by managing provider availability, rescheduling appointments, and supporting patients after hospital discharge.
- Assess patients’ social and cultural contexts, identify unmet social needs, and develop action plans.
- Help patients and caregivers navigate the Texas healthcare landscape and build self-advocacy skills.
- Coordinate transportation and access to telehealth.
- Support patient communication templates and automation, troubleshoot technical issues, and update patient charts in the Case Management System.
- Refer patients to supportive services and community resources, and maintain partnerships with local Texas organizations.
- Use motivational interviewing, provide tailored education, and offer emotional support to families and caregivers.
- Document interactions, time spent, and activities in accordance with Medicare clinical and social integration service standards.
