Requirements
- Bachelor’s degree required; a degree in Public Health, Public Policy, Social Work, or a related field is preferred.
- 5+ years in a client-facing or partner-facing role within community health outreach, nonprofit or public-health consulting, or social-impact strategy.
- Native-level Spanish speaking ability and professional-level Spanish writing skills.
- Experience working with underserved communities in a professional capacity.
- Background in management consulting, nonprofit strategy, or impact investing with a client-facing component.
- Experience translating organizational mission into measurable outcomes.
- Ability to influence partners without authority and build trust toward shared goals.
- Comfort working in complex, multi-stakeholder environments, including health systems, government agencies, and community organizations.
- Comfort using data and analytics tools to track partner health, identify trends, and support recommendations.
- Strong written and verbal communication skills for C-suite and community audiences.
- Occasional travel up to 30–40% of the time.
- Direct experience in community health, FQHCs, public health programs, or social determinants of health initiatives is a plus.
Responsibilities
- Serve as the primary relationship holder for a portfolio of 20–25 CareStation partners.
- Guide partners through onboarding and embed the CareStation into their broader community health strategy.
- Establish shared objectives and mutually agreed goals at the start of each partnership.
- Clarify roles and responsibilities and hold OnMed and partners accountable throughout the program.
- Track and analyze partner-level outcomes data to identify what is working, what is stalling, and where to focus next.
- Run forward-looking, mission-grounded partner reviews.
- Bring partner insights back to OnMed to shape how the roadmap evolves.
- Identify opportunities for partners to expand their commitment and help them build the case internally.
- Document community outcomes across the portfolio to contribute to OnMed’s broader impact narrative.
