Requirements
- Active California Licensed Vocational Nurse (LVN) license, OR Medical Assistant certificate, OR Bachelor’s degree in a related field.
- Minimum of 2 years of telephonic outreach or enrollment experience in a high call volume environment.
- Experience working with complex care clients and ECM Populations of Focus.
- Ability to work Monday through Friday, 8:30am–5:00pm Pacific Time.
- Proficiency with video conferencing, Google Workspace, Slack, and electronic health records.
- Excellent oral and written communication skills.
- Positive interpersonal skills.
- Prior remote work experience.
- Ability to maintain a quiet, secure workspace for member privacy.
- Must be vaccinated against influenza.
Responsibilities
- Engage with prospective clients through daily outbound calls (60-100 calls/day).
- Record all client interactions accurately in the CRM system.
- Convey client preferences and information to the care team for personalized planning.
- Assist clients in completing psychosocial assessments and setting actionable care goals.
- Review medical charts to identify needs and recommend appropriate interventions.
- Apply evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
- Participate in weekly Team Operations Meetings to optimize care delivery.
