Requirements
- 5+ years of experience as a clinical social worker, care coordinator, care navigator, or in a similar field.
- Demonstrated progressive program management experience.
- Direct experience working with Veterans and/or within the VA system.
- 1+ years of experience working successfully in a remote environment.
- Knowledge of VA healthcare, Veteran benefits and resources, behavioral health, and social determinants of health.
- Experience supporting medically and/or behaviorally complex populations.
- Experience documenting cases within a care management or electronic health record system.
- Strong communication, collaboration, and problem-solving skills.
- Experience with motivational interviewing.
- Reside in Texas, Alabama, Mississippi, Louisiana, or Tennessee in the Central Time Zone.
Responsibilities
- Help Veterans understand and navigate available resources, including VA healthcare, benefits, community care, and other services.
- Guide complex Veteran cases, determine appropriate referrals and escalations, and provide direct support when specialized assistance is needed.
- Develop Veteran-specific care coordination workflows.
- Identify opportunities to improve the program.
- Collaborate with care coordination and training leadership to strengthen the team's understanding of VA healthcare, benefits, and resources.
