Requirements
- 3-5+ years of experience in patient onboarding, member engagement, customer service, community health, or healthcare navigation.
- Fully fluent in English and Spanish (verbal and written).
- High emotional intelligence and active listening skills.
- Ability to build immediate rapport and guide members through structured talk tracks.
- Comfortable navigating CRM platforms, virtual communication tools, and scheduling software.
- Ability to hit daily outreach quality and engagement targets.
- High school diploma or GED equivalent.
- Reliable home internet access and a quiet space for taking calls.
- Experience with Medicaid populations is a strong plus.
Responsibilities
- Conduct up to 7 comprehensive, virtual Welcome Visits per day.
- Assess members' baseline needs and educate them on Nadia Care services.
- Identify and escalate urgent medical or social needs to the clinical team.
- Establish warm, trusting relationships with new members.
- Coordinate directly with Maternity Navigators and clinical specialists to ensure smooth care transitions.
- Pass critical intake notes to the core team.
- Maintain data accuracy by logging interactions and scheduling visits in systems like Athena.
