Requirements
- Completion of a Master’s Degree level Family Nurse Practitioner program.
- Current National Board Certification.
- Current, unrestricted license to practice in the Advanced Practice Nurse role in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV.
- Minimum of two years of medically-relevant experience or equivalent.
- Proficiency with information management and technology.
- Effective verbal, written, and electronic communication skills.
- Ability to work without direct supervision and practice autonomously.
- Outstanding organizational skills and ability to multi-task.
- Must meet minimum requirements for remote care delivery, including broadband connectivity and a quiet environment.
Responsibilities
- Provide holistic, evidence-based care including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
- Evaluate primary, acute, chronically ill, and transitional care patients aged 18 months and older.
- Provide patient counseling on pregnancy prevention, STI prevention, safer sex practices, and contraceptive care.
- Document all patient care within an EHR according to clinic policies.
- Coordinate care with internal and external colleagues, including the patient-centered medical home.
- Collaborate within a multidisciplinary team including physicians, paraprofessionals, and pharmacists.
- Work independently to prioritize tasks, solve problems, and advocate for patients.
