- Experience
- Three years of experience within the last five (5) years in a hospice or related health care organization.
Requirements
- Current multi-state RN licensure (currently OH and MI required).
- Minimum three years of experience within the last five years in a hospice or related health care organization.
- Valid driver's license.
- Active auto insurance in the candidate's name.
- Ability to work a part-time schedule of 20 hours per week.
- Ability to work varied after-hours shifts, weekends, and observed holidays.
- Commitment to working every other weekend and two observed holidays per year.
- Capable of meeting all physical demands of the role.
- Compassionate communication skills.
- Strong attention to detail and ability to follow clinical protocols.
Responsibilities
- Triage incoming calls and provide clinical symptom management advice.
- Assess efficacy of recommended treatments and conduct follow-up calls.
- Communicate patient changes and collaborate with physicians and the hospice care team.
- Serve as a clinical resource for weekend and evening teams.
- Reassign patient visits for field staff during off-hours as needed.
- Provide clinical, educational, and emotional support to patients and families.
- Document patient services and updates in HCHB software.
