Requirements

  • Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies.
  • Five years of clinical experience in palliative care, hospice, or working with patients with serious illness.
  • Active Registered Nurse or Physician Assistant license in the State of Texas without any board actions.
  • Board certification as an advanced practice nurse by a nationally recognized certifying agency.
  • Active Unrestricted DEA license.
  • Eligible to maintain adequate malpractice coverage.
  • Ability to support serious illness conversations with members and caregivers.
  • Experience and understanding of value-based contracting with providers.
  • Demonstrated leadership skills and ability to influence others.
  • Excellent communication, negotiation, and relationship-building skills.

Responsibilities

  • Lead internal and joint case rounds to ensure no barriers to care and direct appropriate care plan development.
  • Coordinate services with Network Providers and Longitudinal Care Providers.
  • Review population-level data to identify trends and implement clinical model interventions.
  • Support provider and clinical staff training to ensure knowledge sharing.
  • Support quality management and improvement initiatives.
  • Provide feedback, guidance, and corrective actions to network partners to support their performance.
  • Collaborate across clinical, operational, financial, and technology teams for seamless service integration.
  • Monitor network metrics including utilization, outcomes, cost-effectiveness, and growth.
  • Represent the company in external forums and events.
