Requirements
- Active Compact/Multi-State LPN/LVN license.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with Microsoft Office Suite.
- Ability to work in a fast-paced environment with a focus on phone-based communication.
- Active high-speed Wi-Fi.
- Established home office or HIPAA-compliant workspace.
- Excellent written and verbal communication skills.
- Ability to sit for long periods and take thorough notes in real-time during phone calls.
Responsibilities
- Accept transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients.
- Resolve patient questions and maintain an open dialogue to identify needs.
- Identify and coordinate community resources to support patient care.
- Provide patient education and health literacy training for chronic condition management.
- Assist with medication management, including identifying concerns, supporting adherence, and coordinating refills.
- Ensure timely delivery of critical services such as Home Health, DME, and Home Infusion.
