Requirements
- Master’s Degree in Social Work, Counseling, or related field from an accredited program.
- Active licensure to practice in the state of New York (LMSW, LCSW or equivalent).
- Minimum 1-5 years of experience as a licensed professional.
- Proficiency in medical interpretation and documentation in both English and Spanish.
- Secure, high-speed internet connection supporting video conferencing and cloud systems.
- Proficiency in evidence-based treatment approaches such as CBT, Motivational Interviewing, or Behavioral Activation.
- Experience with behavioral health screening tools (PHQ9, GAD7, CSSRS).
- Technical proficiency in EHR platforms and ability to learn new technology quickly.
Responsibilities
- Provide evidence-based interventions via the Collaborative Care Model to individuals experiencing mild to moderate behavioral health symptoms.
- Support and coordinate care with primary care providers and other service providers.
- Screen patients for behavioral health and substance use disorders using standardized tools like PHQ9 and GAD7.
- Monitor clinical symptoms, treatment side effects, and medication adherence.
- Track patient follow-up and clinical outcomes using registries and electronic health record systems.
- Document patient progress and treatment recommendations in Salesforce and EHR platforms.
- Facilitate care plan adjustments for patients not showing improvement.
- Participate in caseload consultations with psychiatric consultants and clinical supervision sessions.
