Responsibilities
- Engage with patients, referral sources, sales, pharmacy, and nursing to coordinate discharge and secure nursing and delivery.
- Create, communicate, and obtain all required documentation for new referrals.
- Secure patient’s upfront payment and identify financial assistance programs when needed.
- Support ongoing activities for clean claims and denial management.
- Assist supervisor and manager with special assignments as needed.
