Requirements
- Certified Nurse Practitioner in Adult, Family, or Geriatrics.
- Active state licensure in PA.
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics (2 years may be substituted by time in Skilled-Nursing or Long-Term Care).
- Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions.
- Proficiency in motivational interviewing techniques.
- Experience in or appetite for virtual/digital-first care delivery.
- Solid tech proficiency, including familiarity with Google Suite and Slack.
- Excellent written and verbal communication skills.
- Ability to work 24-36 hours/week, including weekend rotation.
Responsibilities
- Conduct telephonic outreach and scheduled video/phone visits for recently discharged patients.
- Perform medication reconciliation and symptom screening.
- Order follow-up labs, medications, and medical supplies as needed.
- Build rapport with patients and explain the care model to improve engagement.
- Coordinate care across hospital, pharmacy, and community provider settings.
- Collaborate with the nursing team and provide input on workflow improvements.
- Meet individual performance targets and team-level population health goals.
