Requirements
- Certified Nurse Practitioner in Adult, Family or Geriatrics.
- Active state licensure in NJ.
- Ability to obtain additional state licensures.
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics (2 years may be substituted by time in Skilled-Nursing or Long Term Care facilities).
- Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions.
- Motivational interviewing skills.
- Experience in or appetite for virtual care delivery.
- Solid tech proficiency, with familiarity in Google Suite and Slack.
- Excellent written and verbal communication skills.
- Availability to work 24-36 hours per week, including monthly weekends.
Responsibilities
- Cover care needs for the Transitions of Care program.
- Conduct telephonic outreach and scheduled video/phone visits for post-hospital discharge patients.
- Perform medication reconciliation, symptom screening, and order necessary labs, medications, or supplies.
- Build trust with patients and explain the Galileo care model.
- Coordinate care across hospital, ED, pharmacy, and community provider settings.
- Collaborate with the internal nursing team.
- Maintain accountability for individual performance targets and team population health goals.
