- Two (2) years of experience as an RN
Requirements
- Associate Degree in Nursing.
- Current licensure as a Registered Nurse in North Carolina.
- Minimum of two years of experience as an RN.
- Clinical knowledge of Minimum Data Set (MDS) system.
- Understanding of Medicare and dual enrollment requirements.
- Knowledge of transition requirements for Long-Term Care medical and pharmaceutical needs.
- Ability to collaborate with community hospitals, EMS, home health, and skilled nursing facilities.
- Effective problem-solving and communication abilities.
- Bachelor’s degree in Nursing (preferred).
- Experience working in a skilled nursing facility (preferred).
Responsibilities
- Ensure individuals transitioning from Skilled Nursing Facilities receive assessment and are linked to Behavioral Health and Medical Homes.
- Coordinate community service planning and participate in virtual care team meetings.
- Assist in the development of participant care plans and discharge plans.
- Provide medication reconciliation and support medication management.
- Monitor results of interventions and treatment to reduce high-risk events.
- Collaborate with clinical teams to educate staff and members regarding network services and supports.
- Maintain timely and complete documentation of all member interactions.
