Requirements
- Residency in Illinois, Indiana, or Wisconsin.
- Active, unrestricted Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Clinical Professional Counselor (LCPC) license in the state of Illinois.
- 6+ years of professional experience in the health care or managed care industry.
- 3+ years of experience working for a managed care health plan.
- Minimum of 2 years of experience in a leadership role managing other leaders or supervisors.
- 3+ years of experience leading care coordination teams to drive measurable outcomes.
- Proficiency in analyzing and interpreting data trends.
- Progressive business consulting and/or operational leadership experience.
- Ability to travel in Illinois to provider offices, government agencies, and Humana locations.
- Reliable internet service (min 25 Mbps download / 10 Mbps upload).
- Dedicated space for working with PHI/HIPAA data.
Responsibilities
- Lead teams of nurses and behavior health professionals responsible for care management.
- Oversee the assessment and evaluation of members' needs to facilitate access to care resources.
- Guide the development, implementation, and evaluation of care management programs to align with regulatory requirements.
- Monitor data sharing among internal and external resources to ensure service integration and continuity of care.
- Facilitate effective communication and collaboration with internal and external stakeholders including providers and payers.
- Present program reports using data analytics to monitor performance and identify trends.
- Champion initiatives for process improvement and innovation in care management practices.
