Requirements
- 4+ years of clinical experience treating patients with CHF, hypertension, and diabetes in inpatient or outpatient settings.
- Active, unencumbered Registered Nurse license with multi-state compact licensure (RN Compact).
- Experience working in a CHF bridge clinic or similar chronic disease management setting.
- Experience working with remote patient monitoring technology and platforms.
- Ability to independently triage abnormal vitals and lead virtual follow-ups for treatment plan changes.
- Availability to work the assigned overnight and weekend schedule: 7:00 PM to 7:00 AM ET.
- Fluency with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and understanding of AI integration in clinical care.
Responsibilities
- Triage inbound patient calls using standardized clinical protocols such as Schmitt-Thompson guidelines.
- Follow up with patients reporting abnormal vitals to assess clinical status and determine triage pathways.
- Monitor patient vitals, symptoms, and labs in real time to identify needs for clinical intervention.
- Manage treatment plans and medication adherence in collaboration with Nurse Practitioners and Physicians.
- Document patient case notes in the Cadence platform and log tasks in Zendesk.
- Respond to clinical questions and escalations by phone and text.
- Re-engage patients who have lapsed on vitals or need rescheduling.
