  • Active Nurse Practitioner license in multiple U.S. states
  • APRN
  • Proficiency with EMR systems and virtual care platforms
  • Background in primary care, chronic care, and medication reviews
  • Strong communication skills
  • Ability to work both independently and within a collaborative care team
  • Flexibility to provide care outside standard business hours occasionally

Responsibilities

  • Conduct Virtual Assessments and Consultations
  • Perform video or phone-based evaluations, reviewing patients' medical histories, medication regimens, and current health status
  • Provide Patient Counseling and Education
  • Deliver guidance on preventive care, chronic disease management, lifestyle modifications, and medication adherence
  • Coordinate Care and Make Referrals
  • Work with health center care teams, specialists, and community-based providers to ensure seamless coordination of care, including necessary referrals and follow-ups
  • Maintain Documentation
  • Accurately document patient encounters and care plans in the electronic medical record system in a timely and compliant manner
