Requirements
- Active Registered Nurse (RN) license in good standing
- 1-3 years of clinical experience, preferably in telehealth, home health, or care coordination
- Experience reviewing patient vitals and clinical data
- Familiarity with EHR systems and telehealth platforms
- Strong communication, documentation, and organizational skills
- Ability to work independently in a remote environment
- Experience in telemedicine or virtual healthcare settings (preferred)
- Multistate licensure or compact license (preferred)
- Fluency in multiple languages (preferred)
- Familiarity with healthcare technology tools and digital health platforms (preferred)
Responsibilities
- Monitor patient vitals from remote monitoring devices (BP, pulse oximeter, glucometer, weight scale)
- Review alerts and escalate abnormal readings according to clinical protocols
- Conduct phone or video check-ins with patients to assess status and provide guidance
- Educate patients on device usage and encourage program compliance
- Collaborate with physicians and healthcare teams to coordinate care and manage chronic conditions
- Maintain accurate documentation in the Electronic Health Record (EHR)
- Recognize urgent situations and escalate appropriately
