Health Advocate (Remote 1099)

Experience in patient advocacy, care coordination, social work, healthcare navigation, or a related field. Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse, often vulnerable patient populations. Exceptional empathy, active listening, and interpersonal skills, enabling you to build trust quickly and maintain meaningful relationships. Strong organizational abilities and clear communication, allowing you to manage multiple tasks effectively. Tech-savvy and capable of quickly learning modern EHR systems, digital collaboration tools, and patient engagement apps. Action-oriented mindset with a commitment to improving patient outcomes and system efficiency.

Guide patients through healthcare challenges, clarifying complex medical information in simple terms, coordinating seamless care, and assisting with insurance paperwork. Create and manage individualized care plans in partnership with Careway physicians, addressing social determinants of health such as transportation, food, home support, and digital access. Proactively identify patient needs, prioritize actions, and coordinate appointments, referrals, and follow-ups. Empower patients by improving their health literacy and self-management skills through education and coaching. Collaborate with our clinical and operational teams, sharing insights to continuously improve Careway’s advocacy-driven care model.
