Requirements:
LCSW, LMFT, LMHC, licensed in Washington State. Preferred experience with substance abuse population. Background in maternal; substance abuse preferred. Experience with adults and adolescents. Familiarity with Medicare and Medicaid procedures. Remote Work Experience.
Responsibilities:
Assesses, plans and implements individualized care strategies. Collaborates with member, family and healthcare providers to develop a plan of care. Conducts individual counseling and group therapy. Identifies and initiates referrals for social service programs. Advocates for members and families. Utilizes approved clinical criteria to assess level of care. Documents member assessments, care plan and referrals. Achieves set goals and Key Performance Indicators (KPIs). Learns the StrongWell model and takes ownership for outcome based care. Participates in interdisciplinary team meetings and utilization management rounds. Promotes responsible and ethical stewardship of company resources. Maintains excellent punctuality and attendance.
