Requirements:

• Master’s degree in Social Work from an accredited institution.
• State licensure as a Licensed Clinical Social Worker (LCSW) required.
• At least 2-4 years of clinical experience, preferably in healthcare settings.
• 2 years of renal experience preferred.
• Previous behavioral health experience and/or hospice or palliative care experience preferred.

Responsibilities:

• Work directly with patients and families (70% of the time).
• Provide behavioral health support to high risk patients and their families, helping them navigate the complexities of kidney disease, and increase coping.
• Connect patients and families with community resources and support services, including financial assistance, behavioral health services, and care needs services.
• Assist patients and families with goals of care conversations, and provide education on the advance care planning process.
• Educate patients, families, and caregivers about available palliative and hospice care services, advocating for patient needs and preferences.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients. Liaise with dialysis clinic staff and outside providers (30% of the time).
