Requirements
- Hold an active, unencumbered Registered Nurse (RN) license in the state of New Jersey.
- Multi-state Compact Licensure is preferred.
- Have a minimum of 1–2 years of clinical experience in home health, triage, or remote care management.
- Demonstrate clinical triage, assessment, and critical-thinking skills.
- Use Google Workspace and EHR care management systems proficiently.
- Maintain a dedicated home office with high-speed internet meeting HIPAA privacy standards.
Responsibilities
- Conduct virtual clinical intakes, health screenings, and wellness checks with patients and caregivers.
- Perform telephonic triage and clinical assessments for patient calls, directing care using established protocols.
- Review and update care plans in alignment with physician orders and state regulations.
- Monitor Remote Patient Monitoring (RPM) data and vital trends, flagging clinical changes for the care team.
- Serve as a primary virtual point of contact for patients, families, attending physicians, and home care staff.
- Maintain accurate, timely clinical documentation in EHR platforms.
- Participate in virtual case conferences, team huddles, and quality assurance reviews.
