Requirements
- Hold an active NBHWC health coaching certification.
- Have at least 1 year of experience as a health coach.
- Have experience providing patient education and support to people managing chronic conditions.
- Be fluent in Spanish.
- Be ready to work remotely, including maintaining stable high-speed Wi-Fi.
- Be fluent with AI-assisted clinical tools such as Gemini, Open Evidence, or comparable platforms.
- Be able to discuss credibly with providers and patients where AI supports care delivery and where human judgment remains essential.
- Be able to adhere reliably to the stated schedule.
Responsibilities
- Conduct outbound telephonic outreach and provide one-on-one coaching to patients managing chronic conditions.
- Coach patients on behavior modification, nutrition, physical activity, and self-management using motivational interviewing and SMART goals.
- Assess patients’ health status, lifestyle behaviors, nutritional habits, and readiness to change.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
- Carry out care delivery tasks under appropriate clinical supervision and in accordance with clinical policies and protocols.
- Monitor patient progress, treatment-plan adherence, and health outcomes through check-ins and remote monitoring tools.
- Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Identify barriers to care and connect patients with resources or internal support pathways.
- Collaborate with clinicians and patient support teams using the Cadence Platform, Google Workspace, and Slack.
