Requirements
- Hold an active compact multi-state LPN/LVN license in the state where you currently reside.
- Have at least 5 years of clinical experience as a Licensed Practical Nurse.
- Be fluent in Spanish.
- Be ready to work remotely, including having stable high-speed WiFi.
- Be comfortable using AI-assisted clinical tools such as Gemini, Open Evidence, or comparable platforms.
- Use clinical judgment when applying AI-assisted tools to support patient interactions and clinical context.
- Experience providing patient education and support to people managing chronic conditions is preferred.
- An active California LVN license is highly preferred.
Responsibilities
- Conduct outbound telephonic outreach and provide one-on-one coaching to patients managing chronic conditions.
- Help patients follow personalized care plans through education and coaching on behavior modification, nutrition, physical activity, and self-management.
- Assess patients’ health status, lifestyle behaviors, nutritional habits, and readiness to change.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
- Carry out care delivery tasks according to clinical policies and established protocols under appropriate clinical supervision.
- Monitor patient progress, treatment-plan adherence, and health outcomes through check-ins and remote monitoring tools.
- Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Identify barriers to care, including social needs, and connect patients with resources or internal support pathways.
- Collaborate with clinicians and patient support teams using the Cadence Platform, Google Workspace, and Slack.
