- One or more years of experience working with the geriatric or certified disabled population in case management/care coordination, Home Care, Group Homes, Nursing Homes, TCU or Assisted Living settings preferred

Requirements
- Hold a Minnesota Licensed Social Worker credential or a current Minnesota Registered Nurse license.
- Have a valid driver’s license.
- Have access to a private and compliant home office space.
- Work independently.
- MNChoices experience is highly preferred; paid training is available.
- One or more years of experience working with geriatric or certified disabled populations in case management/care coordination, Home Care, Group Homes, Nursing Homes, TCU, or Assisted Living settings is preferred.
- Demonstrate creative problem-solving skills.
- Demonstrate appreciation for working with diverse populations.
- Communicate effectively with strong verbal skills.
- Have excellent interpersonal and customer service skills.
- Demonstrate compatibility with Bluestone’s mission and operating philosophies.
- Read, write, speak, and understand English.
Responsibilities
- Contact new enrollees to complete initial assessments.
- Review members’ current health needs and identify their goals.
- Develop individualized care plans and transitions of care.
- Connect members with resources and services.
- Complete required documentation.
