- 2+ years of clinical experience (acute, home health, ambulatory, or case management.)
Requirements
- Maintain an active LVN/LPN license in good standing.
- A California license is preferred.
- Have 2+ years of clinical experience in acute care, home health, ambulatory care, or case management.
- Have experience with EMR/EHR systems and clinical workflows.
- Demonstrate strong telephonic and virtual communication skills.
- Be able to manage multiple priorities in a fast-paced remote environment.
- Demonstrate the ability to work within scope and escalate appropriately.
Responsibilities
- Coordinate day-to-day care activities across a virtually managed patient population.
- Support patient throughput, care coordination, and longitudinal case management activities across the acute care episode delivered in the home.
- Execute real-time workflows and provide case management support aligned with clinical plans, regulatory standards, and program protocols.
- Work under the direction of an RN/provider and within LVN licensure scope.
- Escalate appropriately and avoid clinical decision-making beyond scope.
