Requirements
- Hold a California Nurse Practitioner or Physician Assistant license in good standing.
- Have at least 2 years of nephrology experience in outpatient or telehealth settings.
- Have strong clinical documentation skills.
- Be comfortable practicing autonomously in a remote care environment.
- Have a reliable internet connection.
- Be familiar with telemedicine technology.
Responsibilities
- Evaluate, diagnose, and manage chronic kidney diseases.
- Conduct follow-up visits and provide longitudinal care for established patients.
- Document patient encounters accurately and promptly in Advanced MD.
- Collaborate with specialists and care team members to coordinate patient-centered care.
- Follow clinical protocols, quality standards, and regulatory requirements.
- Develop, implement, and adjust treatment plans with supervising physicians as required by California regulations.
- Order and interpret laboratory and diagnostic studies.
- Provide patient education and counseling on lifestyle, medication adherence, and disease management.
