Requirements
- Be a board-certified nurse practitioner or physician assistant.
- Maintain active state licenses in coverage areas.
- Nurse practitioners should preferably hold FNP or AGNP certification.
- Be able to obtain and maintain multistate licenses in coverage areas, as required.
- Have experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format; this is preferred.
- Provide video visits from a quiet, private setting with a neutral background and broadband connectivity with video and audio enabled.
- Be able to uphold health plan member privacy per CVS Health guidelines.
- Have effective verbal, written, and electronic communication skills.
- Be able to work without direct supervision and practice autonomously.
- Have proficiency with information management and technology.
- Be able to consistently provide 40 hours of weekly availability.
Responsibilities
- Conduct online video visits with health plan members in a private space with adequate internet connectivity.
- Review members’ medications and medical histories during approximately 30-minute visits.
- Educate members about their health and answer their questions.
- Provide 40 hours of availability weekly and consistently provide 320 or more capacity units per month.
- Meet a strong visit-completion rate, consistently pacing 220 or more completed visits per month.
- Maintain training, affiliation, and compliance with video visit requirements.
- Be trained and credentialed for health plan clients and lines of business in primary coverage states.
- Participate in regular manager, team, and clinical meetings.
- Meet Signify Health operational, clinical, and member experience key performance indicators and metrics.
