Requirements
- Bachelor of Science in Nursing (BSN) required.
- Unrestricted active Registered Nurse (RN) license required, with eligibility to obtain additional state licenses as needed.
- Minimum of 7 years of nursing experience, including at least 5 years in primary care with result follow-up experience.
- Outpatient experience required.
- Related certification required or to be obtained within 12 months of hire: AMB-C, CCM, CMGT-BC, OCN, AOCN, AOCNS, or another related certification.
- Strong organizational skills and ability to multitask in a fast-paced environment.
- Strong verbal and written communication skills, with empathy and active listening.
- Comfort using video chat, electronic health records, and other telehealth tools.
- Regular availability during Pacific Time business hours.
- Professional fluency in English and Spanish, including medical Spanish, strongly preferred.
- An active Nurse Licensure Compact (NLC) multi-state license is preferred.
Responsibilities
- Provide triage, care coordination, and education by phone and asynchronous messaging.
- Conduct clinical and needs assessments to identify and address patient needs.
- Conduct telephone visits with patients reporting symptoms and coordinate follow-up care or escalate when needed.
- Follow up on abnormal cancer screening results, explain next steps, and coordinate appointments through completion of the appropriate workup.
- Review annual patient engagement surveys for changes in screening needs or new clinical needs.
- Collaborate with Care Advocates and other care team members on clinical and non-clinical support needs.
- Develop clinical protocols and best practices for care coordination from screening through survivorship.
- Help address symptom management, care escalations, provider referrals, and planning for in-person care appointments.
- Participate in care team sessions to monitor member progress and develop targeted support plans.
- Maintain detailed documentation of patient encounters.
