Requirements
- Hold Medical Assistant certification (CMA, RMA, or equivalent).
- Have 2+ years of MA experience in a clinical, telehealth, or care management setting.
- Have prior eClinicalWorks (eCW) experience and proficiency; it is the system of record for documentation, CCM time logging, and care plan management.
- Have working knowledge of CCM, RPM, or chronic disease management programs.
- Have cardiology experience.
- Familiarity with CCM/RPM CPT coding, including 99490, 99439, 99457, and 99458, is preferred.
- Bilingual Spanish is a plus.
Responsibilities
- Review the RPM vitals dashboard daily and act on abnormal blood pressure, heart rate, weight changes, and missed readings.
- Contact patients about abnormal vitals by text first, follow up by phone by the end of the day, and escalate urgent concerns to the care manager.
- Log asynchronous CCM time in eClinicalWorks for billable care coordination activities.
- Keep documentation audit-ready and consistent with CMS billing guidelines.
- Onboard newly enrolled patients into the RPM program, providing device education and troubleshooting.
- Manage a panel of patients enrolled in RPM.
- Manage ordering, supply chain, and inventory for connected RPM devices.
- Manage inventory of ancillary materials delivered in patient kits, including exercise equipment and education materials.
