Requirements
- BS in Pharmacy required, PharmD preferred.
- Active pharmacy license in the state of residence.
- Ability to obtain a NJ state license within 6 months of hire.
- 2+ years of experience in pharmacy distribution and packaging.
- Exposure to geriatrics and/or chronic care management.
- Strong technical skills for navigating proprietary pharmacy software.
- Demonstrated problem-solving, conflict resolution, and project management skills.
- Knowledge of state and federal laws and regulations, including HIPAA, Fraud, Waste, and Abuse.
- Ability to work weekends, holidays, or on-call as needed.
- Available for occasional travel for meetings and conferences.
Responsibilities
- Serve as a point of contact for patients, families, and healthcare providers to address medication-related problems and pharmacotherapy needs.
- Explain care plan options, medication usage, side effects, and contraindications.
- Ensure patient care decisions, billing, and distribution are properly documented using proprietary software.
- Plan and maintain procedures for assessing, compounding, packaging, and labeling medications according to legal requirements.
- Oversee pharmacy technicians regarding medication reconciliation, inventory, and storage compliance.
- Provide final clinical review and approval for prescription orders to ensure correct dosage and patient safety.
- Use professional judgment to make recommendations to care providers to enhance patient safety and outcomes.
