Requirements

  • Four-year college degree.
  • Minimum two (2) years of experience and/or training with hospice.
  • Must have a HIPAA-compliant home office.
  • Proficiency in Microsoft Office Suite (including Excel and Teams).
  • Ability to read, write, and effectively communicate in English.
  • Strong verbal and written communication skills.
  • Critical thinking and multitasking abilities.
  • Willingness to learn hospice regulatory standards and HCHB software.
  • Experience in medical office, customer service, or intake/inside sales preferred.
  • Licensed Practical Nurse (LPN) certification preferred.
  • Availability to work scheduled evenings and weekends.
  • Ability to work at least one major holiday and two minor holidays per year.

Responsibilities

  • Receive and coordinate hospice referrals from physicians, hospitals, and community resources.
  • Act as the lead coordinator for all client referral and admission activities.
  • Collaborate with departments to gather billing and clinical preadmission information.
  • Verify insurance eligibility and obtain necessary authorizations.
  • Explain hospice benefits, philosophy, and services to clients and their families.
  • Enter and audit client data accurately within the HCHB system.
  • Coordinate care transitions, including durable medical equipment and comfort kits.
  • Follow up on pending referrals and maintain relationships with referral sources.
