Requirements
- MD or DO degree.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics.
- Active medical license in the state of Georgia (multi-state licensure is a plus).
- Credentialed with major insurance plans (UHC, Aetna, Humana).
- Strong interest in preventive care, senior health, and chronic condition management.
- Tech-savvy with experience or comfort in practicing via telehealth platforms.
- Collaborative and mission-driven mindset.
Responsibilities
- Conduct routine telehealth visits with Medicare patients focusing on E/M and Chronic Care Management.
- Develop and oversee personalized care plans for patients with ongoing conditions.
- Collaborate closely with an integrated care team including nurse case managers, NPs, and Community Health Workers.
- Maintain thorough and accurate documentation within the Electronic Health Record (EHR) system.
- Ensure ongoing compliance with clinical quality measures.
- Engage patients in preventive care strategies to minimize avoidable health complications.
