Requirements
- Fully bilingual in Spanish and English languages.
- Current and unrestricted Registered Nurse (RN) license in the state of GA.
- Minimum of 3 years of patient care nursing experience, preferably in primary care, preventive care, or telehealth.
- Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.
- Proficient in electronic health records (EHR) systems and remote medical documentation.
- Strong knowledge of Medicare guidelines, telehealth practices, and preventive care strategies.
- Strong organizational skills and attention to detail.
- Ability to work independently and manage time effectively in a remote work setting.
- Reliable internet connection (minimum 100 Mbps download, 10 Mbps upload) for video calls and system connectivity.
Responsibilities
- Conducting Medicare Annual Wellness Visits via telehealth in accordance with CMS guidelines.
- Virtually reviewing and updating patient medical histories, including medication lists, immunization records, and family history.
- Assessing patients’ current health status, vital signs, cognitive function, and chronic disease risk factors.
- Providing patient education on preventive measures, lifestyle changes, and community resources.
- Developing and updating personalized prevention plans.
- Collaborating with the supervising physician on documentation and follow-up using electronic health record systems.
- Identifying HCC codes requiring recapture reporting to CMS.
- Facilitating virtual referrals to appropriate healthcare services.
- Monitoring patient progress and adherence to personalized prevention plans.
