Requirements
- Current Board of Certification (BOC) for the Athletic Trainer certification.
- Active, good-standing state athletic trainer licensure.
- Experience working directly with patients in physical therapy, rehabilitation, orthopedic, sports medicine, or musculoskeletal clinical settings.
- Strong clinical judgment and ability to apply structured protocols.
- Experience conducting outbound telephone outreach and navigating sensitive patient conversations.
- Proficiency in interpreting clinical records and navigating electronic medical record (EMR) systems.
- Excellent written documentation and verbal communication skills.
- Ability to work independently in a remote environment.
- Experience with healthcare data, utilization review, or population health preferred.
- Experience with virtual or technology-enabled healthcare programs preferred.
Responsibilities
- Review claims and EMR data to identify members with recurring fee-for-service utilization related to musculoskeletal conditions.
- Evaluate clinical information using established protocols and program criteria.
- Conduct outbound telephone outreach to assess member status, barriers to care, and re-engagement interest.
- Route appropriate members into clinical care pathways and document all interactions.
- Coordinate with multidisciplinary clinical teams to ensure smooth handoffs.
- Track outreach, contact, and conversion rates to report on program performance.
