Requirements
- Degree in a health-related field.
- Minimum of three years of experience helping people manage their health, ideally in a clinical or remote health coaching setting.
- Fluent in both English and Spanish (written and verbal) is required.
- Knowledge of cardiac rehabilitation and cardiac procedures/medications.
- Experience with counseling or education regarding disease management.
- Understanding of recent nutrition and exercise trends.
- Experience working with an elderly patient population.
- Proficiency in clinical documentation.
- Strong motivational interviewing and goal-setting skills.
- Detail-oriented with a team-first and resourceful mindset.
Responsibilities
- Conduct initial assessments for patients enrolling in the virtual cardiac recovery program.
- Educate patients on program benefits and the importance of risk factor modification.
- Coach individuals through electronic chat messaging and video sessions.
- Problem-solve and support individuals' current and evolving health goals.
- Adapt in-clinic best practices to a remote-care delivery model.
- Complete individualized treatment plans including nutrition, psychosocial, fitness, and comorbidity assessments.
- Translate evidence-based best practices for chronic condition management into an empowering patient approach.
- Motivate and encourage patients during initial visits.
- Review medical history and provide basic guidance on cardiac procedures and medications.
