Requirements
- Must have Certified Diabetes Care and Education Specialist (CDCES) certification.
- Registered Dietitian with active licensure.
- Bachelor’s Degree in a health-related field (e.g., Dietetics, Health Education, Wellness, Health Promotion).
- 3+ years of coaching experience.
- Demonstrated experience in wellness areas such as diabetes, weight management, stress management, nutrition, or physical activity.
- Proficiency in MS Office applications including Excel, Word, PowerPoint, and Outlook.
- Strong communication, problem-solving, and interpersonal skills.
- Ability to work effectively and exercise sound judgment in challenging situations.
Responsibilities
- Conduct comprehensive evaluations of member health status and risks using clinical tools and data analysis.
- Establish personalized plans of care based on health assessment data and individual support needs.
- Utilize motivational interviewing and active listening to ensure member engagement and promote healthy lifestyle changes.
- Monitor care plans in collaboration with the member and their attending physician.
- Identify quality of care issues through established organizational channels.
- Ensure compliance with internal policies, regulatory requirements, and accreditation guidelines.
