Requirements

  • Current CMA or CCMA certificate issued by a recognized credentialing body.
  • 2 years of healthcare experience.
  • 1 year of telephonic or call center customer service experience.
  • Experience working in a medical practice or healthcare setting.
  • Familiarity with EHR platforms for documenting patient interactions and scheduling.
  • Experience and training in motivational interviewing and patient education.
  • Comfort discussing serious illness, chronic disease, and end-of-life topics.
  • Strong organizational skills with the ability to manage a high-volume outreach workload.
  • Proficiency with Microsoft and Google applications.
  • Access to a reliable internet connection.
  • Strong written and verbal communication skills.
  • Exceptional interpersonal skills, including conflict resolution and de-escalation techniques.
  • Willingness to work Monday through Friday, 8:30am–5pm EST.

Responsibilities

  • Conduct targeted telephonic outreach to patients on provided lists, following standard messaging and maintaining customer service standards.
  • Confirm service area eligibility and complete structured screening forms capturing demographic information, chronic disease history, and indicators of functional decline.
  • Educate patients and families on palliative care, services, and program expectations, translating complex clinical concepts into accessible language.
  • Schedule eligible patients for initial visits with a nurse practitioner and coordinate logistics for a smooth transition to care.
  • Act as a liaison between patients and the care team, responding to patient questions and concerns in a supportive manner.
  • Maintain confidentiality and accuracy in all patient documentation in accordance with HIPAA standards.
  • Coordinate enrollment efforts across regions and track outreach activity to encourage referrals and census growth.
  • Identify outreach trends and barriers, providing feedback to the market and business development team to improve program strategy.
  • Maintain a collaborative working relationship with the market team and practice leadership.
