Requirements
- Current Registered Nurse (RN) license, or, in Maine only, Licensed Practical Nurse (LPN) who completes the required SAMHSA training for an X-DEA license.
- Bachelor's degree in Nursing preferred.
- At least 5 years of direct patient-care experience in addiction medicine, behavioral health, or a related clinical setting.
- Strong understanding of care coordination, case management, population health, and interdisciplinary care delivery.
- Knowledge of healthcare regulations, HIPAA requirements, and professional standards.
- Strong data-analysis skills, including the ability to interpret registries and outcomes.
- Proficiency with electronic medical records (EMR), Microsoft Office, or Google Workspace.
- Ability to build trust and rapport with diverse patient populations.
- Reliable internet service (minimum 50 Mbps download/10 Mbps upload) for remote work.
- Ability to meet background-screening, credentialing, licensing, and CPR/Basic First Aid requirements.
Responsibilities
- Maintain accurate member registries for specialized populations to track outcomes and support care-team decision-making.
- Facilitate regular registry reviews with interdisciplinary teams including counselors and medical providers.
- Analyze population-level data to identify quality improvement opportunities and initiatives.
- Manage assigned member caseloads through telephone, SMS, and telemedicine interactions.
- Perform care coordination, screenings, and structured assessments for complex medical needs.
- Support medication management by monitoring adherence, side effects, and treatment effectiveness.
- Educate members on physical and mental health conditions and navigation strategies.
- Facilitate and document treatment-plan updates in collaboration with clinical providers.
