Requirements
- Active LVN/LPN license in good standing (CA license preferred)
- 2+ years of clinical experience (acute, home health, ambulatory, or case management)
- Experience with EMR/EHR systems and clinical workflows
- Strong telephonic/virtual communication skills
- Ability to manage multiple priorities in a fast-paced, remote environment
- Demonstrated ability to work within scope and escalate appropriately
Responsibilities
- Work under direction of RN/provider and within LVN licensure scope
- Support patient throughput, care coordination, and longitudinal case management activities
- Coordinate day-to-day care activities across a virtually managed patient population
- Integrate real-time workflow execution with case management support
- Ensure patients receive timely, appropriate, and coordinated services aligned with clinical plans and program protocols
