Requirements
- Active NP license in good standing in home state (IMLC/eNLC compact license strongly preferred).
- Board certification through ANCC or AANP as a Family, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care NP.
- Minimum 2 years of post-licensure clinical experience in primary care, internal medicine, endocrinology, cardiology, or geriatrics.
- Comfort with telehealth or digital care delivery and screen-based, asynchronous-forward environments.
- Strong documentation discipline and familiarity with EHR systems.
- Excellent independent clinical judgment.
- US residency in any of the 50 states or US territories.
- Willingness to obtain additional licensure in target states within 90 days of start.
- Spanish language proficiency is a plus.
- Experience with value-based care, CMS quality measures, or ACO/care management programs preferred.
- Familiarity with Cardiovascular-Kidney-Metabolic conditions and GLP-1 therapy management.
Responsibilities
- Conduct virtual medical evaluations via video, phone, and asynchronous forms, confirming diagnoses and assessing baseline metrics.
- Create and manage holistic, evidence-based care plans for patients with conditions such as obesity, dyslipidemia, hypertension, ASCVD, and diabetes.
- Act as the primary prescriber in Full Practice Authority states and under Collaborative Practice Agreements in non-FPA states.
- Manage device-triggered Red Alerts and triage patients for ER referral or medication titration adjustments.
- Document all patient interactions in the EHR with accurate ICD-10 coding and prepare PCP care coordination reports.
- Collaborate with the Medical Director to refine titration algorithms, clinical protocols, and scalable operational workflows.
- Provide feedback on app-based care tools and remote monitoring integration to improve patient experience.
