Requirements
- Doctor of Pharmacy (PharmD) or equivalent advanced degree.
- Current licensure as a Pharmacist.
- Must be NH-licensed.
- Strong knowledge of specialty medication pharmacology and pharmacy regulations.
- Proficiency with Microsoft Word, Excel, and PowerPoint.
- Ability to use database and reporting applications.
- Ability to identify barriers to medication adherence and implement improvement plans.
- Strong verbal and written communication skills.
- Ability to motivate and coach patients.
- Experience with oncology or other specialty disease states preferred.
- Experience with EPIC/QS1/WILLOW software is a plus.
Responsibilities
- Review and interpret clinician patient care notes and assessments for prior authorization approvals.
- Develop and refine disease and drug-specific clinical protocols to promote evidence-based care.
- Monitor patient adherence using PDC reports, assessment data, and mobile platform metrics.
- Evaluate EMR data for drug-disease interactions, medication selection, labs, and vitals.
- Perform medication therapy management (MTM) including therapy monitoring and documenting interventions.
- Conduct pharmacist consultations, motivational interviewing, medication synchronization, and medication reconciliation.
- Oversee daily activities of pharmacy technicians and verify accuracy of coordination slips and PA logs.
- Identify and resolve insurance-related issues and facilitate financial assistance.
