- 2+ years of experience in behavioral health, social services, or a related field.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams).
- High School Diploma or equivalent required.
- Ability to navigate various healthcare technology tools.
- Strong problem-solving and engagement skills.
- Demonstrated ability to maintain professional relationships and communicate effectively.
- Access to a private, dedicated workspace to perform job duties.
- Separate care arrangements for dependents during work hours.
Responsibilities
- Conduct annual Health Risk Surveys to support member Individual Plans of Care.
- Escalate identified health, safety risks, or service needs to the assigned Care Manager.
- Execute care coordination activities delegated by the Care Manager within established timeframes.
- Report quality of care issues to the Care Manager or Associate Manager.
- Advocate for member care coordination needs and drive effective solutions.
- Engage members using motivational interviewing techniques to encourage lifestyle changes.
- Document case management activities in compliance with regulatory and accreditation guidelines.
