Requirements
- At least 2 years of health care experience, preferably in behavioral health.
- Active, unrestricted license as an LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD, or PsyD.
- Experience working with persons with severe and persistent mental health concerns, serious emotional disturbances, substance use disorder, and foster care.
- Knowledge of whole person care principles, chronic health conditions, and discharge planning.
- Proficiency with Microsoft Office suite and clinical data entry platforms.
- Valid and unrestricted driver's license, reliable transportation, and auto insurance.
- Ability to travel locally 25-40% of the time as required.
- Certified Case Manager (CCM) preferred.
- Field-based care management or home health experience preferred.
Responsibilities
- Complete comprehensive behavioral health assessments and determine eligibility for case management.
- Develop and implement care plans in collaboration with members, caregivers, and healthcare professionals.
- Conduct telephonic, face-to-face, or home visits as required.
- Monitor care plan effectiveness, document interventions, and track goal achievement.
- Promote service integration including behavioral health and long-term services and supports (LTSS).
- Facilitate interdisciplinary care team meetings and collaboration.
- Use motivational interviewing and clinical guideposts to support member care.
