• Technical aptitude – Microsoft Office Suite
• Excellent written and verbal communication skills
• Epic Experience (preferred)
• Bilingual (preferred)
• Additional state licensures (LPN) (preferred)

• Manage patient census and address care gaps
• Identify and coordinate community resources
• Provide patient education on chronic condition management
• Perform medication management and coordination
• Assist in timely service delivery to patients
• Manage referrals and appointment scheduling
