Requirements:

• High school diploma or GED required.
• Must have a valid NYS driver's license and reliable vehicle.
• Strong computer skills and ability to learn new technologies.
• Empathy, compassion, and patience.
• Superior communication skills and professionalism.
• Ability to work in a HIPAA compliant environment.

Responsibilities:

• Monitor all alerts and respond to patient communications.
• Schedule and conduct audio-visual calls with patients.
• Work with a Registered Nurse to report abnormal values or concerns.
• Support participants' overall wellness and address social determinants of health.
• Troubleshoot technology issues and provide device education during home visits.
